About The Role

Are you ready to be the person who can positively affect the outcome of a young person’s life?

As a Health and Wellbeing Practitioner, you’ll be right at the heart of delivering impactful, youth-centred support across local schools, youth hubs, and community settings. This role is all about meeting young people where they are—whether that’s in the classroom, youth justice system, or local drop-in centre—and offering them non-judgemental, holistic help around risky behaviours including substance use and sexual health.

You’ll hold a caseload of young people and deliver a range of evidence-based interventions, from brief advice to structured psychosocial support. You’ll also contribute to health promotion through school sessions, community outreach, and targeted group work—empowering young people to make informed choices about their wellbeing. Working closely with our Engagement and Participation team, you’ll help shape education sessions, peer-led programmes and ensure young people’s voices remain central to service design.

Whether you’re delivering harm reduction advice, participating in safeguarding meetings, or providing one-to-one care planning, you’ll play a vital role in reducing risk, building resilience, and improving life chances for children and young people.

This isn't just a job—it's a chance to do something that matters.

About You

You’re someone who sees beyond the behaviour and into the potential. Whether it’s supporting a young person at a youth club, leading a group session in school, or offering guidance over the phone—you bring empathy, energy, and a commitment to helping young people thrive.

You’ll need:

  • Experience working with children, young people or young adults in community settings
  • Confidence facilitating group work and engaging vulnerable or at-risk groups
  • A strong understanding of safeguarding, risk-taking behaviours, and developmental needs
  • Experience working in multi-agency settings and contributing to care planning
  • The ability to manage a varied caseload across different community and clinical locations
  • A relevant qualification in youth work, psychology, counselling, social work or mental health (e.g. NVQ Level 3 or equivalent)

If you also bring experience in advocacy, delivering professional training, mentoring, or using evidence-based approaches, that’s a real plus—but we’re far more interested in your values, energy and commitment than a perfect CV.

If you’re motivated by impact and want to help young people shape brighter futures—this could be the role that makes that change!
