Are you ready to be the person who can positively affect the outcome of a young person’s life?
As a Health and Wellbeing Practitioner, you’ll be right at the heart of delivering impactful, youth-centred support across local schools, youth hubs, and community settings. This role is all about meeting young people where they are—whether that’s in the classroom, youth justice system, or local drop-in centre—and offering them non-judgemental, holistic help around risky behaviours including substance use and sexual health.
You’ll hold a caseload of young people and deliver a range of evidence-based interventions, from brief advice to structured psychosocial support. You’ll also contribute to health promotion through school sessions, community outreach, and targeted group work—empowering young people to make informed choices about their wellbeing. Working closely with our Engagement and Participation team, you’ll help shape education sessions, peer-led programmes and ensure young people’s voices remain central to service design.
Whether you’re delivering harm reduction advice, participating in safeguarding meetings, or providing one-to-one care planning, you’ll play a vital role in reducing risk, building resilience, and improving life chances for children and young people.
This isn't just a job—it's a chance to do something that matters.
