About The Role

Could you help make that first step feel a little less scary and a lot more hopeful?

As a Triage and Assessment Practitioner, you’ll be the first point of meaningful contact for children, young people and families accessing our service. Working as part of a supportive, multidisciplinary team, you’ll carry out holistic assessments to understand each young person’s needs and ensure they’re matched with the right support at the right time.

You’ll take the lead on triaging new referrals, conducting comprehensive assessments by phone, and liaising with families, schools and professionals to develop effective care plans. You’ll also help manage the assessment rota and contribute to the ongoing development of efficient, inclusive pathways into care. Every decision you make will directly impact a young person’s experience of the service and their journey to better emotional wellbeing.

This role is perfect for someone who thrives on detail, values relationships, and can balance empathy with structure. If you’re passionate about early help and prevention, and want to be part of a service that puts children and young people first, we’d love to hear from you.

This is a fixed-term role until 31 March 2027.

Be the steady start to something better. Apply Today!

About You

You thrive on finding clarity in complexity. Behind every referral, you see a young person needing someone to listen, understand, and guide them to the right support. You bring professionalism, compassion and calm, and you're driven by the belief that early, effective intervention changes lives.

To succeed in this role, you’ll need:

  • A qualification in mental health or in working with children and young people (e.g. Youth Work, Social Work, Psychology, Counselling)
  • At least one year’s post-qualification experience
  • Experience delivering holistic assessments for children and young people
  • Work with vulnerable groups and in multi-agency settings
  • Strong understanding of safeguarding, risk assessment, consent and Gillick competency
  • Time management skills and ability to work under pressure
  • Confident IT and case recording skills
  • A collaborative, flexible attitude with excellent communication skills

Although this is a remote/home based role you will need to occasionally travel to Northallerton and the York Head Office therefore will need to have a UK driving licence and access to a vehicle

Desirable criteria include a CBT-informed approach, experience in organising team duties, and knowledge of evidence-based practice or child development that will give you an even stronger start but aren't dealbreakers.

Are you the person who helps change the course of a young persons life? Sounds like you…join us!
