About The Role

Are you ready to be the person who helps a young person feel seen, heard and genuinely supported?

As a Health and Wellbeing Practitioner, you will be out there making it count in schools, youth spaces and the community, connecting with young people where life is really happening. This role is about more than support. It is about showing up, building trust, and being the steady hand that helps a young person take back control.

You will hold a caseload of young people, delivering everything from quick check ins to structured, one to one support. You will offer guidance around substance use, sexual health and other risky behaviours, and play a big part in our school education and outreach work. You will also take part in the duty system, answering calls and texts, making sure help is there when it is needed most.

You will shape group sessions, lead on a specialist topic you care about, and help grow our peer education programme. Whether you are planning with a teacher, meeting with a social worker, or helping a young person feel hopeful again, you will be making a real difference every single day.

This is a role with purpose, heart and impact. If you lead with integrity, value every young person, and are ready to bring consistent, solution focused support where it counts, this is where it starts.

About You

You are someone who brings more than just a job title.

You bring empathy, energy and genuine commitment, especially when young people are struggling to trust others or believe in themselves. You understand that trust takes time, progress is not always linear, and that sometimes the most powerful thing you can do is simply show up and stay present.

You will bring:

  • Experience working with children, young people or young adults in a community or education setting

  • Confidence in running group sessions and supporting young people through difficult or uncertain moments

  • A strong understanding of safeguarding, risk taking behaviours and how young people grow and adapt

  • Experience of working across services and contributing to multi agency support plans

  • The ability to manage a busy and varied caseload while staying calm, focused and organised

  • A Level 3 qualification or equivalent in youth work, social care, mental health or a related area

You may also bring experience in peer education, advocacy or training, but more than anything we are interested in your values and mindset. If you are someone who works with integrity, treats each young person as an individual, and stays solution focused and dependable, then you are exactly the kind of person we want in this team.

This is your opportunity to make your work matter. If you believe in young people, act with purpose and want to be part of something that makes a difference every single day, then now is the time to step in.
